CALL 911 - provide location.
PAGE CODE RED - provide location.
Follow Fire Plan.

R.A.C.E.:

  1. Rescue - Rescure all individuals, including staff, patients or visitors, nearest the fire. Close doors behind you.
  2. Alarm - Activate the nearest fire alarm pull station.
  3. Confine - Attempt to confine the fire by closing doors between you and the fire. Touch the door prior to entering the facility to ensure you are not walking into an inferno.
  4. Evacuate - Leave the area as per Evacuation procedure.

When directed by Manager or Designate, announce:
"CODE RED LOCATION ALL CLEAR"
Repeat this announcement 3 times.

PAGE CODE BLUE. - state Adult or Pediatric with Location.
Code Blue Team Respond - CPR/AED.

  1. The first team member to get to the scene will begin with CPR, and all subsequent individuals will be designated to help.
  2. Designated person will call 911 and ensure area is cleared of people.
  3. Designated person will get AED.
  4. Non designates are expected to avoid the area and keep hallways clear.

When directed by Manager / Designate announce:
“CODE BLUE LOCATION ALL CLEAR”
Repeat this announcement 3 times.

PAGE CODE GREEN - with location

Remove as many patients as possible, including their family members, from the affected areas. While you may want to save as many lives as possible, it is important to consider your own safety too. Do not put yourself in a life-threatening situation unless absolutely necessary.

Evacuate the least-intensive patients first as follows:

  1. Evacuate ambulatory patients.
  2. Remove wheelchair-bound patients.
  3. Patient is in the middle of medical procedures should be moved as soon as it is reasonably safe to do so. If the procedure is wound dressing or other minor procedure, stop, cover the wound and begnin respective evacuation protocols.

If patient family members are present, ask them to please help with getting their loved ones out of the clinic.

Your team should meet in the predetermined muster stations to take attendance.

When directed by Manager / Designate announce:
"CODE GREEN LOCATION ALL CLEAR"
Repeat this announcement 3 times.

PAGE CODE YELLOW - with location

Activated by any staff member after noticing a patient is missing from their area.

  1. Nearest staff secure ALL exits and no one is allowed to leave the clinic.
  2. Provide description of patient (gender, clothing, and description), last location known
  3. Staff in that area conduct a search in all assigned patient rooms, staff areas and hallways.
  4. Report negative or positive findings to staff that initiated the code.
  5. If patient not found, call 911.

When directed by Manager / Designate announce:
"CODE YELLOW LOCATION ALL CLEAR"
Repeat this announcement 3 times.

PAGE CODE PINK - with location

Activated by any staff member after noticing a child is missing from their area.

  1. Nearest staff secure ALL exits and no one is allowed to leave the clinic.
  2. Provide description of infant/child (gender, approximate age, clothing, and description), last location known, and description of abductor.
  3. Staff conduct a search in all assigned patient rooms, staff areas and hallways.
  4. Require anyone matching the description to please remove oversized clothing, like heavy jackets , check all bags/packages for the missing child.
  5. Notify another staff person of anyone matching the description prior to confronting the individual.

Use any means necessary, without putting yourself or the presumed person of interest at risk for violence or injury, to prevent the individual from leaving the facility.

Call for help if an individual is actively trying to leave a facility with a child during a Code Pink. Report negative or positive findings to staff that initiated the code. If child not found, call 911.

When directed by Manager / Designate announce:
"CODE PINK LOCATION ALL CLEAR"
Repeat this announcement 3 times.

PAGE CODE WHITE

  1. Activate the GREEN button and wait for assistance.
  2. Nearby staff to assess situation; call 911 with location and details if situation escalates or a weapon detected.
  3. Remove other patients, staff, or visitors from danger.
  4. Non designates are expected to avoid the area.

When directed by Manager / Designate announce:
"CODE WHITE LOCATION ALL CLEAR"
Repeat this announcement 3 times.

PAGE CODE GREY

Activated by management to inform all personnel that a potentially serious weather emergency exists.

Staff and patients to move away from windows and glass doors and gather in the waiting rooms.
Management will monitor situation and provide further direction.

When directed by Manager / Designate announce:
"CODE GREY ALL CLEAR"
Repeat this announcement 3 times.
